Northern Light Health’s Acadia hospital is seeking a Behavioral Health Home Coordinator. This full time position is an integrated position located at Blue Hill Primary Care.
The Behavioral Health Home Coordinator works within a multidisciplinary team, providing comprehensive case management services to improve mental health, and in conjunction with the primary care provider, improve medical health to adults with serious and persistent mental illness.
Requirements include: High School Diploma/GED
